Separate discs into folders, but only if there's multiple discs?

I’d like to have my multiple-disc albums have a subfolder for each disc within the album. For example,

-Album X
–Disc 1
—Song 1
—Song 2
–Disc 2
—Song 1
—Song 2

And I want regular (one-disc) albums to be sorted as such:

-Album Y
–Song 1
–Song 2

Is this possible?
